As I understand it, they're getting this past Medium because they're using Cyrillic characters in the username. They look exactly the same to us, but the Unicode (what a computer sees is different.
It's an understood problem (UTR-36, UTS-39)- and I'm a little surprised that the coders at Medium haven't implemented a fix. I suspect that converting Unicode to ascii, then comparing might be a quick-and-dirty that’s ‘good enough’.
Also, it’s OK to select ‘also block this user’ when you’re reporting spam. You’re not blocking Dylan (and as he mentions you can click on the username to be sure).
