I'm surprised there's no vaccine for it in African nations. It's an orthopox, so immunity to vaccina (cowpox) should protect against it as it does for smallpox. The traditional smallpox vaccination (with vaccina) has a small, but not non-existent rate of side effects (pericardium) - and the pustule fluid is infective. There's a new vaccination (requires 2 shots) with a replication-disabled vaccina that has a much lower rate of side effects. Maybe that's the one that isn't available in Africa?
