I'm a little surprised insulin levels (and maybe IGF) aren't on your list. People with T2DM have higher cancer rates - even the ones with their weight under control - which suggests that persistent high insulin levels could be a factor. People with T2DM have insulin resistance for a long, long time before the beta cells can't keep upping insulin levels to maintain normal glucose levels to overwhelm the resistance in skeletal muscle.
I've read of some in vitro work showing that insulin on established cancer cells is like gasoline on the BBQ.
