Agreed that PWV is a good metric for arterial compliance.
Speaking as someone who's been taking spironolactone to counter hyperaldosteremia for the last 35 years or so. I had the option to simply have the offending adrenal out but passed on surgery as it looked to impact my training schedule - and then eventually inertia. My challenge of late is balancing the dose to get the lowest BP I can while still avoiding side effects (mostly cramping from what seems to be low Mg - supplementing with that seems to have helped). So far calcium scan shows zero, curious what PWV might show...
